New Patients

Welcome to POST PT! If you are a new patient to one of our clinics, please complete the New Patient Registration form before your first appointment. When you book your initial evaluation, we also recommend scheduling your first four appointments to help ensure a convenient and consistent treatment schedule. If you are scheduled for a telehealth session, please click here for instructions for your first appointment. For in-person visits, your therapist will meet you at the door to our waiting room at the start of the session.

We strongly recommend that you contact your insurance provider to verify your benefit information and to determine the cost per session. Our office does not have access to the financial agreement between patients and insurers, so it’s important to understand your financial responsibility ahead of time. We ask for payment at the time of service and process co-pays using the credit card information provided in your new patient paperwork and encrypted to your file.
